CAREmasters homehealth is your solution! Our job is not to provide the same care for every client, but rather to develop a custom care plan that addresses each person's unique STORY.

 

Home care is a service that assists those in need to continue living and celebrating life from the comfort of their own home. Home care is a model of care that includes both professional and informal support networks that include family, neighbors, and friends. To remain independent, these individuals construct your care team which work together to meet your goals and expected outcomes.

 

All services can be arranged for any frequency of time, from 1-24 hours of care daily, or on a respite or temporary basis.

 

There are NO restrictions to care!

  • No daily minimums
  • No restrictions to time services can be provided - services can be requested for any time of the day or night
  • No additional charges for holidays
  • No additional charges for weekends
  • No additional charges for mileage
  • No additional charges for visits lasting less than 4 hours

 

WHAT TO EXPECT DURING AN INITIAL INTAKE ASSESSMENT

A free, no obligation, comprehensive care assessment is completed by a CAREmasters RN for any client requiring or requesting personal care (hands-on) assistance or by a Qualified Supervisor for any client requiring or requesting homemaker, companion, or concierge services. The assessment visit can range from 1-2 hours in length and primarily occurs in the client's place of residence. The RN or Supervisor will review the following with the client and/or client's representative at the time of the visit:

 

  1. Client's medical information (to include medical diagnosis(es), medication(s), current health care plan, and medical history)
  2. Environment and current resources
  3. Lifestyle (to include social activities and history, exercise regimen, sleeping patterns, and daily routine)
  4. Diet and nutritional requirements
  5. Current resources and family involvement in care
  6. Need for additional services, products or equipment from third party partners
  7. Funding sources and funding options

 

In addition to a thorough review and examination of the items listed above, a complete safety evaluation is completed and includes:

 

  1. Fall risk evaluation
  2. Full house safety evaluation
  3. Remote care technology & safety alert systems in place
  4. Back up plan review
